Systematic review and independent validation of genetic factors of radiographic outcome in rheumatoid arthritis identifies a genome-wide association with CARD9.

Impact: the Viatte lab, working with our international collaborators, has conducted a systematic review and validation of all know genetic associations with disease severity in rheumatoid arthritis outside the HLA and demonstrated that very few genetic determinants of disease susceptibility are shared with genetic determinants of disease outcome. This finding supports the principle that different genetic factors underlie transition to different clinical stages of autoimmune diseases. This contribution has important implications for the choice of genetic markers to include in the calculation of Polygenic Risk Scores (PRS) for stratified medicine in general.

Impact: this study, co-led with Darren Plant and funded by our industrial partner Bristol Myers Squibb, represents an important step in assessing the potential of genetic stratification for patients with rheumatoid arthritis. We confirm that genetic factors located within the HLA are associated with response to treatment, but we show that this association is not treatment-specific, and therefore cannot be used in a clinical setting to stratify patients into treatment response categories.

Impact: This was the first systematic study conducted across all known genetic markers of rheumatoid arthritis susceptibility to show that some of these markers are shared, while others are specific to different serotypes of rheumatoid arthritis (heterogeneity).
